The posterior auricular vein originates from a superficial venous plexus that is found on the lateral aspect of the head. It arises from small veins behind the auricle superficial to the temporal fascia. These vessels also communicate with the tributaries of the occipital and superficial temporal veins.

The posterior auricular vein descends behind the auricle alongside the posterior auricular artery. Once the vein reaches the sternocleidomastoid, it further descends on its superficial surface.

Near the apex of the parotid gland, the posterior auricular vein unites with the posterior division of the retromandibular vein, forming the external jugular vein (Read more!).

Along its way, the posterior auricular vein receives a stylomastoid vein from the middle ear and some posterior auricular veins that drain the auricle.

Overall, the posterior auricular vein collects deoxygenated blood from the muscles and skin of the scalp behind the ear, superior and posterior to it.
