The central groove of the second maxillary premolar is shorter compared to the first maxillary premolar. Therefore, the mesial and distal pits at each end of the central groove are located very close to each other, and sometimes they merge, resulting in only one central pit.

These pits appear as sharp-pointed apexes of the distal fossa and mesial fossa that can also be found on either end of the central groove. The second maxillary premolar also presents with the mesial triangular fossa and distal triangular fossa.

Several supplemental grooves can be found on the occlusal surface of this tooth. They give a wrinkled appearance to the surface.

The second maxillary premolar does not contain a mesial marginal groove.

Besides the central groove, the second maxillary premolar has the following grooves:

Mesiobuccal groove - goes from the mesial pit towards the buccal cusp;
Mesiolingual groove - passes from the mesial pit towards the lingual cusp;
Distobuccal groove - extends from the distal pit towards the buccal cusp;
Distolingual groove - goes from the distal pit towards the lingual cusp.
