The dimensions of the second maxillary molar are smaller compared to the first maxillary molar tooth. Also, the line angles of this tooth tend to be less sharp, and the buccolingual length is larger than the mesiodistal length.

The second maxillary molar presents with four cusps - mesiobuccal, distobuccal, mesiolingual and distolingual. All cusps have the same named cusp apexes.

The buccal surface presents with the buccal cervical ridge.

The distal cusps of the second maxillary molar tend to be not so well-developed. Moreover, this tooth can lack the distolingual cusp and have only three cusps. The second maxillary molar usually does not have the fifth cusp.

The second maxillary molar has three roots - mesiobuccal, distobuccal and lingual. They are less divergent compared to the first maxillary molar tooth. Sometimes they are fused or appear parallel to each other. The second maxillary molar has a longer root trunk, and the lingual root does not contain a lingual root depression.
